EBM in three sentences.
Practices bill their services for people with statutory insurance (GKV) through their Kassenärztliche Vereinigung (KV), using the uniform value scale, the EBM.1
Every service has a fee schedule item (GOP) with a number of points. Points times the point value give the amount. The federal basis is the orientation value: 12.7404 cents a point in 2026.1, 2
Whether an item can be billed depends on its service description: duration, number of contacts, interval, an approval or a diagnosis. These are exactly the conditions that slip on a full day.
Where fees can be left behind
Everyday billing codes, easily missed conditions.
Not exotic items, just a GP practice’s daily work. What makes the service is usually in the note. It can still be missing from the bill.

EBM questions
Which EBM billing codes does meda look for?
Documented but unbilled services, the common GP patterns first: consultations, wound care, chronic care, check-ups. Every suggestion carries the line of the note that supports it.
Does 03100 replace the chronic-care surcharge?
Only for a narrow group: patients aged 18 to 74 with exactly one of these conditions: hypothyroidism or autoimmune thyroiditis, lipid metabolism disorder, essential hypertension without hypertensive crisis, idiopathic gout; exactly one prescription drug for it and, for the first billing, contacts with the same practice for that condition in three of the last four quarters. Everyone else stays on 03000 and, if its conditions are met, 03220/03221.6 The 03100 check shows in eight questions whether a patient qualifies.
How often can I bill 03230?
Per full ten minutes of consultation, within the practice’s consultation volume. The volume is 64 points per treatment case. What matters is that the note shows how long the talk took and why.3, 4
What happens in a KV plausibility check (Plausibilitätsprüfung)?
Every approved billing code stays linked to its note and the supporting ICD-10-GM diagnosis. When the KV asks, the code, the note, the diagnosis and the approval are kept together.
Is this upcoding?
No. Only billing codes your note supports, and the doctor approves every one. Correct billing, with evidence.
Does meda change anything in our PVS?
Only what you approve. Suggestions stay in the review; nothing enters your PVS unless you approve it.
